With Help 364, employees who receive a diagnosis for a chronic disease during one doctor’s visit have access to guidance and support the other 364 days of the year. Participants work individually or in groups with Nurse Wellness Coaches who leverage the life-dimensional approach, helping them make lifestyle changes to live well with their chronic conditions.
These skilled nurses review health data from a variety of sources to ensure they are getting life-dimensional support to make behavior changes to help alleviate chronic disease distress. From education and guidance to support and accountability, Nurse Wellness Coaches help give employees what they need to develop better behaviors for managing their conditions and improving their lifestyles.
Program includes:
- An assessment to get a 360° view of a person’s health
- Regular sessions focused on education around a person's chronic disease(s)
- Sessions available via phone, video, or onsite
- Ongoing tracking of biopsychosocial vital signs
- Access to recommended on-demand learning in between sessions
